What is Transition?

Transition is the way that people move from Children’s Services to Adulthood. This may involve working with adult health services or may result in independent management of your health condition

This happens step by step.

We know this can be a difficult and stressful time, but planning the way ahead reduces worry.

What does a Walsall Case Manager do?

  • We meet with you and your family to find out what you need.
  • We help you find support and information
  • Put you in touch with people who can help you

Referral Criteria

We offer support and advice to young people who have a physical impairment and are moving from children’s services to adult services.  We offer this to all young people from Walsall from Year Nine onwards who are generally in mainstream education.

We offer support to Young People (14-25 years) who have:

*A long term physical health condition, generally in local mainstream schools with mild or no associated Learning Disability.

*A long term condition such as asthma, eczema, and allergies – that are known to the Specialist Community Nursing Team.

 

We help young people to gain access to services available within health, leisure and social care.
